How do I change my name when I divorce?

If when you divorce you wish to revert back to using the name you had prior to your marriage, you can usually include the name change official as part of your legal divorce for no additional cost.  This order is a standard part of the divorce decree language in most states.  

If you decide to restore your previous name after your divorce is final, you can usually request the modification through a form to the court.  Each state is different, so consult the court clerk for the correct form to use.
